How can I make the switch to a vegetarian diet and also ensure that I am getting adequate nutrition?

I'm considering making the switch a diet containing mainly raw vegetables and fruit (and probably still eat eggs, bread, and occasionally some salmon / tuna) but am having trouble planning the first to buy the whole market food and meal plan each week in a way that ensures I'm getting most vitamins and minerals, proteins, etc (I will be taking B12 supplements and I can guarantee, etc, but still). Is there a place on the web that is Map of the week with my recipes and ideas daily nutrition? Is there a meal plan or can I sign something with a focus on this type of lifestyle?
